I was on board the Willie Wood from 1971-1973.  The shoulder sleeve patches were not in use at that time.  I found this one while cleaning out one of the filing cabinets in the Weapons Office.  I asked a couple of the older hands about it and none of them could remember ever seeing anyone wear it.  This would mean this item would have had to have been in use sometime prior to 1969.
